Lightyear Calling Card Services

• By submitting this electronic authorization, I represent that I a) am an authorized representative of the Company or Card Holder, b) have approval to authorize the submitted calling card information on behalf of the Company or Card Holder and bind the Company or Card Holder to all of the following terms and c) have read, understand and agree with the terms hereof.
• Submission of this form authorizes Lightyear to provide us (me) with a free calling card. We (I) understand that the primary use of the calling card is for long distance calling when away from office (or home) and that we will billed for any charges incurred while using our (my) Lightyear calling card.
• We (I) acknowledge that this Agreement is subject to approval by Lightyear, Inc. We (I) authorize Lightyear to verify all listed information for credit purposes.
• We (I) accept responsibility for any and all costs, charges or expenses that are incurred on any calling card issued by Lightyear, Inc., including but not limited to charges or expenses that are incurred as a result of a loss, theft, misuse or abuse of the card. We (I) understand that the card may not be transferred or assigned. We (I) agree to immediately report any suspected loss, theft, misuse or abuse of the card to a representative by calling 800-320-6829.

Terms of Authorization

Lightyear 1+ Services

• By submitting this electronic authorization, I represent that I a) am an authorized representative of the Company or Subscriber, b) have approval to authorize the submitted information on behalf of the Company or Subscriber and bind the Company or Subscriber to all of the following terms and c) have read, understand and agree with the terms hereof. I represent that we (I) are the Subscriber for the telephone number(s) listed herein and that I have authority to change long distance carriers.
• Submission of this form appoints Lightyear, Inc. as our (my) primary long distance carrier for equal access service including interstate, intrastate, intralata (where available) and international service. We (I) appoint Lightyear, Inc. to act as our (my) agent in order to effectuate the change(s) and authorize it to handle on our (my) behalf all arrangements, including ordering, changing and/or maintaining our (my) service. We (I) understand that my local telephone company may assess us (me) a nominal service fee to switch my long distance carrier. We (I) understand that only one long distance carrier my be designated as our (my) primary interstate carrier for any one telephone number, but that we (I) may have the ability (depending on our (my) location) to select separate long distance carriers for other types of service.
• We (I) acknowledge that this Agreement is subject to approval by Lightyear, Inc. We (I) authorize Lightyear to verify all listed information for credit purposes.

Lightyear Toll Free Services

• By submitting this electronic authorization, I represent that I a) am an authorized representative of the Company or Subscriber, b) have approval to authorize the submitted information on behalf of the Company or Subscriber and bind the Company or Subscriber to all of the following terms and c) have read, understand and agree with the terms hereof. I represent that we (I) are the Subscriber for the toll free number(s) listed herein and that we (I) are the exclusive end user of these toll free number(s)..
• As the end user Subscriber for this (these) Toll Free number(s), we (I) authorize Lightyear, Inc. (UDI01) to act as or become the Responsible Organization (RespOrg) for this (these) Toll Free Number(s) and to act on our (my) behalf to transfer, if applicable, RespOrg functions to Lightyear.
• We (I) acknowledge that this Agreement is subject to approval by Lightyear, Inc. We (I) authorize Lightyear to verify all listed information for credit purposes.
